Admission Process

Steps from outpatient/emergency care to admission

  1. Eligible Patients

    Specialized rehabilitation, dialysis, infection isolation (CRE/VRE/CPE), oncology supportive care, auto-insurance/industrial injury, and high-acuity patients

    Older adults with chronic disease, dementia patients, and patients requiring postoperative or post-injury recovery care


Admission process

Admission Registration Steps

From admission decision to ward transfer, follow the steps below in order. Please review required documents and counters in advance.

    Admission consultation

    Consult on same-day admission, scheduled admission, or admission related to procedures/tests.

  2. 2
    Clinical evaluation and admission decision

    After outpatient or emergency evaluation, the attending physician decides on admission.

  3. 3
    Admission registration

    Submit identification and the admission consent form at the registration desk (1F).

  4. 4
    Room assignment

    Room assignment depends on same-day bed availability and may differ from your preference.

  5. 5
    Ward transfer and orientation

    After receiving the caregiver pass and orientation guide, proceed to the assigned ward.


Admission Type Guidance

Arrival times, counters, and required documents vary by admission type. Please identify your type and prepare in advance.

Same-day admission

  • Arrival time: by 9:00 AM on the day of consultation
  • Counter: 1F Admission Desk (Administration Office)
  • Required documents: ID and consultation registration slip

Scheduled admission

  • Arrival time: by 10:00 AM on scheduled date
  • Counter: 1F Scheduled Admission Desk
  • Required documents: ID, reservation receipt, admission consent form

Procedure/Test-related admission

  • Arrival time: by 2:00 PM on the day before procedure/test
  • Counter: nurse station in assigned ward
  • Required documents: ID, procedure consent form, pre-test results

Room assignment may vary based on same-day bed availability and may differ from your preferred room. Thank you for your understanding.


Admission Checklist

Preparing the items below in advance will help speed up your admission process.

Required documents

  • Government-issued ID (resident card/driver's license)
  • One of the following: transfer note, medical note, or referral letter
  • Admission consent form (can be completed at the desk)
  • Life-sustaining treatment (DNR) consent form (can be completed at the desk)
  • Current medication list and instructions

If tests were performed at another hospital

  • MRI/CT CD and radiology report
  • Swallowing fluoroscopy report
  • EMG and bone density test reports

Personal items

  • Personal hygiene items (toothbrush, toothpaste, towel)
  • Indoor slippers (non-slip)
  • Mobile phone charger
  • Personal essentials

Admission Desk Hours

Admission desk hours vary by day and time. Please check the schedule below before visiting.

Emergency admissions can be processed at the emergency administration desk during weekday clinic hours. For evening/weekend scheduled admissions, please confirm in advance with the administration team.

Discharge Process Timeline

Discharge is divided into preparation and billing. Reviewing the timeline in advance can reduce waiting time.

Before discharge - physician consultation and discharge decision

The attending physician confirms discharge and informs caregivers. If you need certificates or records, please request them one day in advance.

Discharge guidance - Receive billing guide

Receive the billing guide and discharge prescription from the ward administrator.

Discharge settlement - payment and billing

Pay at the 1F payment desk by card, cash, or bank transfer. If interim payment was already made, only the remaining balance is charged.

Discharge medication - final instructions

Receive prescribed medication, confirm your next outpatient appointment, and review post-discharge precautions.

Practical discharge tips

  • Request certificates and medical records by the day before discharge.
  • On discharge day, morning billing is usually faster.
  • Parking fee settlement can be processed together with billing.

Interim billing for long stays

For long-term admissions, interim billing is available. Contact the ward administrator for guidance.

Payment methods

Credit/debit card, cash, and bank transfer are all accepted. Use the 1F payment desk or the self-service kiosk.

Certificate issuance

Certificates, admission/discharge confirmations, and medical record copies are issued at the 1F certificate desk. ID is required; proxy pickup requires an authorization letter.
